We accept: Debit Cards for Health Savings Accounts / Flex Spending
Accounts, credit cards- Visa/Mastercard, debit cards, checks and cash.
There is a discount for payment received at the time of service.
We request keeping a credit card authorization on file to bill co-pays,
deductibles and other services not covered by your insurance.
To cancel your appointment, please notify us at least 24 hour before
your appointment. There is a $50 charge for missed appointments.
We have a Non-Covered Benefits Fee. The fee is $25 per month for
individuals or $40 per month for families. You may choose to have the
fee billed monthly or quarterly.
There is a $50 charge for returned or NSF checks.
Lab fees: On site labs include Rapid Strep, Pregnancy and Stool for
occult blood. Routine labs are referred to Quest Labs and STAT labs
can be obtained through Verdugo Hills Hospital or Glendale Adventist
Hospital Emergency Rooms. Outside labs will bill you or your insurance
directly. Imaging studies are referred to Glendale Adventist Medical
Center or Verdugo Hills Hospital.
Currently, we do not stock vaccines. Vaccines are available at the
Glendale Health Center or the local CVS or Minute Clinic.

Patients who are admitted to Glendale Adventist Medical Center will be
cared for by hospital specialists using procedures in place at that
medical center.
